Anticoagulation regimens for acute ischemic stroke reduce but do not eliminate risk; in most cases recurrent stroke does not mean the regimen was "wrong"! But the urge to always "do" or "change" something in medicine can sometimes cause harm if not data-driven.

From 1994 to 2000 in NYC, I had never seen a Hunt-Hess 5 patient survive to 90 days with a good outcome Once we started coiling and offering intentional critical care to the the sicker, bombed out grade 5's, 50% were alive at 90 days, and many were in remarkably good shape
