The strong link between #delirium & mortality is not prominent enough in the minds of most acute care staff In this study a positive delirium test (4AT) predicted nearly 50% MORTALITY AT ONE YEAR

There is a real urgency to get it right for older adults across health & social care. These resources based on Ageing & Frailty standards enable that improvement journey. We need to work as a focused & integrated teams to deliver this around Scotland #frailty

In the #AssistedDying debate, “choice” is constantly invoked, as it was in today’s debate, as if it trumps every other consideration. But choice isn’t exercised in a vacuum. It’s an uneven principle, influenced by mood, inequality & awareness of options.

It’s time for an end to torso-based medicine. It’s not practical or possible for all older people to get a CGA. But they should all get an assessment of cognition and function. What is their 4AT? And how are they mobilising today (compared to normal) should be as common as NEWS

"Just a bit muddled" - avoiding saying "delirium" - doesn't cut it. It's unprofessional. Euphemisms don't help patients or families families prepare or understand. Use the right word: DELIRIUM. #delirium

“…palliative care is about living, joy, fun, thinking about your legacy and working with a team to get on top of the big challenges. It’s only a little bit about dying. Let’s call it assisted living.” Great piece on palliative care by Prof. Mark Taubert

Shifting care into the community without District Nurses is like trying to run aviation without air traffic control You can do it but the dangerous uncoordinated mess that will emerge is a pretty sobering thought Nuffield Trust report coming soon. nuffieldtrust.org.uk/resource/the-l…

The importance of time to listen, really listen cannot be underestimated but also the time to act on what is heard. So many times the act of listening is misconstrued as the act of asking a question and writing down the answer...that's it. That's not it.

When you hoard knowledge, you create scarcity around your expertise. When you share it strategically, you create abundance and authority. The traditional academic believes exclusivity equals value. The academic entrepreneur understands that accessibility multiplies value.
