Very interesting study. Stem cell transplantation as a successful therapy for a subset of IBD patients. Would certainly help a lot of people undergoing a lot of suffering.

Yet another reason not to overuse antibiotics that are not necessary. Anaerobic empiric therapy not needed if infection suspected in an area without anaerobes! And certainly de escalation as appropriate.

Would be important for Secretary Kennedy and health admin to see this. For all people claiming vaccines aren’t RCT tested vs placebo, take a look. The data is all there…with 260 trials and 2.5million patients randomized. There is simply no argument to overwhelming data.

It’s been well demonstrated that beta lactams (now specifically cefazolin) should be used over vancomycin for MSSA and here’s another meta analysis. Using vanco would be an inferior treatment strategy.

Important to have prophylaxis in very high risk patients. Pemibivart seems to work very well for Covid, with relatively low side effect profile.

Not surprising that cocci cases are increasing, am seeing it very frequently. With climate change cases continue to rise. Important for early diagnosis, and eventually, newer treatments as resistance develops.

Never blindly follow consultant recs. This removes critical thinking from people who may know the pt and case better. Typically a full discussion should be had unless case is relatively straightforward - though even still communication is key.

Great analysis. Exercise is good but the effect size cannot be greater than cancer therapy itself! Important to point out physical fitness helps improve outcomes in many areas of medicine. Frailty prior to transplant has been independently linked to poor outcomes, as an example

Would first determine if truly a UTI and not asymptomatic bacteriuria - most commonly seen in such resistant bugs because of prior over treatment. If true UTI - multiple options incl cefiderocol, ceftol-tazo, fosfomycin. Newer drugs on horizon as well.
