Nice redo post-infarct case - given prior inferior infarct we explored the epicardial space and found nice WADLs and channels seen from multiple points of view: sinus, RV pacing, and LV pacing. @davilandre, Fritz Stabenau , BIDMC VT Center , #ablateVT

Infarct VT w/ U-shaped WADL in sinus w/ perfect match with a “dead-end” CT wall thickness channel (inHEART). No surprise the VT isthmus was there. A substrate dead end doesn’t necessarily mean bystander! VT is in 3D! BIDMC VT Center Enrico Ferro, MD @davilandre Roderick Tung

Peter Zimetbaum, MD and I had a great outcome for our patient BIDMC w/ #pacemaker +MSSA bacteremia w/ 2x2cm right atrial mass: #ICE imaging essential to direct #alphavac & remove mass with one single manual aspiration. Given PFO, this greatly enhanced safety of lead extraction!

CVCT - CardioVascular Clinical Trialists: It was great to present our JAMA SAFE-IVC analysis with U.S. FDA, and see so much interest from the RCT community on how we can use trial data and "real-world evidence" #RWE as complementary tools. Thank you Dr. Robert M. Califf & Shirley Wang for moderating!

Which map here is VT and which is substrate? A key to mapping fixed lines of block during VT ablation to make the WADLs best match the VT boundaries is to strategically choose a pacing site close to the presumed exit. BIDMC VT Center #ablateVT 1/2🧵
