“The severity of anemia, need for transfusions, and clinical morbidity in β-thalassemia are closely tied to the degree of imbalance between α-globin and β-globin chains.” Excellent β-thal NEJM review nejm.org/doi/full/10.10…

Remember urine dipstick recognizes negatively charged albumin but DOES NOT react to light chains (which are + charged). So even patients with high amounts of Bence-Jones proteinuria will have neg dipstick unless spilling albumin too. Do that 24 hour urine! #mmsm #amyloidosis

I can’t even tell you guys how much I love these @BUHemeOncFellows. They got through this year with their shoulder to shoulder with their heads down but their eyes on the future. So proud.

Acquired hemophilia due to an autoantibody to FVIII, is a diagnosis that experienced hemostasis and thrombosis physicians can make from the end of the bed. Once seen never forgotten. It is the subject of the weekly Case Record discussion in the NEJM. tinyurl.com/288ne32p

Here is a palliative care #TipsForNewDocs: check out open access NEJM video & article on leading "family meetings". It's 10 minutes long so can be watched right before a meeting for just in time learning (and not behind a paywall). #medtwitter #HAPC nejm.org/doi/full/10.10…

My conclusions: 1) folate def reasonably prevalent in a Boston safety-net population; keep checking when indicated for these pts 2) important demonstration that study population assessed *matters deeply* - cannot always extrapolate from non-safety net to safety net

Honored to have been offered the opportunity to present 40 years of natural history data in AL amyloidosis from BU Amyloidosis Center at #ASH21, under the mentorship of Vaishali Sanchorawala. Causes of mortality shift over disease course, with fewer cardiac/sudden deaths in late disease.
