I think current National Institute for Health and Care Research training structures are made for 5-to-6 year training programmes, not 7-to-8 (esp where the training is top-heavy, as in paeds/O&G SST). Hard to navigate post-PhD in my experience.

Another clinical academic friend (Dr who does teaching and research alongside NHS work) is moving into a solely clinical post Clinical academics are not nurtured in the UK, despite longer training (10+ yrs after medschool) and over-delivering for their universities and hospitals

Current system is a huge waste of talent, time and future potential. It doesn't have to be this way. Clinical academics in training often think they are only a success if win the 'big fellowships but with nuanced institutional and programme support, talent can stay Academy of Medical Sciences
