It started out by the observation in the #HOT_ICU_trial that the 100 pts enroled with #COVID_19 had benefits from the lower vs. higher oxygenation target (published as a post-hoc analysis onlinelibrary.wiley.com/doi/epdf/10.11…

#HOT_ICU_trial was swiftly transformed to #HOT_COVID_trial planned to enrol 780 pts The trial was terminated early after the winter 22-23 and the omicron variant because of very few pts in ICUs with severe hypoxemia At that time 726 patients had been enrolled

Of the 726 pts randomised most patients were analysed 69% were males Age 65 years 24% on invasive ventilation 13% on non-invasive Most with P/F-ratios < 100 Randomisation occured 3 hours after ICU admission

The protocol, i.e., lower vs higher oxygenation targets while in ICU, separated the 2 allocation groups

Primary outcome - days alive wo life-support at 90 days - was 80.0 days in the lower O2-target group vs. 72.0 days in the higher group (P = .009; bootstrapped adjusted mean difference, 5.8 days [95% CI, 0.2-11.5 days]) There were no meaningful sub-group differences in effect

The effect of the primary outcome was supported by sensitivity analyses and those of secondary outcomes

The conclusion of #HOT_COVID_trial was In adult ICU patients with COVID-19 and severe hypoxemia, targeting a Pao2 of 60 mm Hg (8 kPa) resulted in more days alive without life support in 90 days than targeting a Pao2 of 90 mm Hg (12 kPa) Not that we hope to see COVID pts in ICU
