CT: "Normal" atlanto-occipital joint space. MRI: Unstable craniocervical dissociation injury (same pt.) 🔑: Don't solely rely on the joint space or "lines" to exclude CCJ trauma. Exclude 1. Prevertebral 2. Supradental 3. Retroclival hematomas too!

35 year old man with HIV with ear pain followed by hearing loss, facial weakness, and vertigo. MRI showed THIS. CSF with normal cell count, cytology, and flow cytometry. Diagnosis? I'll give you 100 guesses and you will still fail.

Important from Francis Deng, MD #Radiopaedia2025 re: neurovascular compression of CN VII - from the root exit point to the proximal cisternal segment. Don't forget that the initial segment of CN VII is the 'attached segment' which lies against the pons, all centrally myelinated.

1/My hardest thread yet! Are you up for the challenge? How stroke perfusion imaging works! Ever wonder why it’s Tmax & not Tmin? Do you not question & let RAPID read the perfusion for you? Not anymore!

🤷‍♂️A 23-year-old woman presents with a painless, progressive swelling in the left submandibular region and an intraoral bulge. No dysphagia, voice changes, or neurological deficits. 👨‍⚕️What diagnosis would you consider for this deep neck mass?

👶A 6-month-old boy is referred due to increasing head circumference. Neurological exam is unremarkable. Serum AFP and β-HCG are negative. Surgery is performed the following day. Histology confirms the diagnosis. 🕵️‍♂️⁉️What would you suspect?

This week’s SPIN-POV: In suspected CNS relapse of ALL, don’t stop at the meninges or parenchyma—look closely at the marrow. Hidden infiltration, especially in clivus or skull base, may be better seen on DWI and ADC than conventional sequences.

1/Time to go with the flow! Hoping no one notices you don’t know the anatomy of internal carotid (ICA)? Do you say “carotid siphon” & hope no one asks for more detail? Here’s a thread to help you siphon off some information about ICA anatomy!
