Frailty #virtualward will have a mix of various disciplines - acute frailty, community geriatrics, primary care, ambulatory care, anticipatory care, EOL care, care home medicine to name a few 😃. Keen for new members of our team to develop skills in all of them under one service

Would like to give a shout out to Karen Kelly and Sue O’Neill, social workers on UCR and helping us with our frailty #virtualward patients. Achieved poc qds and 2 mobile night calls in 2 hours today and crisis placements. Wonderful to have such compassionate colleagues Wirral Community Health&Care NHS Foundation Trust

Great recommendations. I would add, where appropriate clinically and available locally also Urgent Community Response and Hospital at Home/ Virtual Wards.

Those who aren't geriatricians can assume that CGA is an assessment process, whereas the initiated are aware that CGA is a multidimensional assessment process with actions. I think the key part of the message is "actions"

601 CT head scans. 5 bleeds, 4 of which predicted by high risk status. So 596 people no benefit from scan and many will have had unseen harm- a trip to ED, fear or pain in scanner, a hospital stay, ‘deconditioning’ aka ‘got worse in hospital’ - we need to change this!
