LLMs have always done this - it is a product of the token process and the memory limitations. Sam Coates Sky - LLMs remain unbelievably useful and powerful but not if you use them like this, check them in this way, or rely on them in this way!

Before highlighting as a solution, wouldn’t it be better to look into the quality of patient ratings and how they relate to outcomes to ensure they could be accurate markers of clinical care and not solely experience markers?
