Oral bioavailability of ketamine is 8-20%, no way to get to anesthetic serum concentrations as seen in his autopsy with oral dosing. IV dose 1.5 weeks prior for depression not causal. Sublingual 25mg lozenge may have higher bioavailability 30-40% but far from anesthetic at 25mg

I can’t believe these new Medicare trigger point coverage policies. I am shocked that a lack of published evidence explicitly showing safety from ultrasound use makes it now investigational to use ultrasound. Blind injection in to a pec minor or piriformis muscle is negligent.
