🏥 It's important to assess for both #frailty & #delirium at the front door. 📕 Study: Clinical Frailty Scale (CFS) & 4AT have complementary value in predicting hospital admission in older #ED patients 🔗 sciencedirect.com/science/articl… #CFS #4AT

Families usually know before clinicians: "Dad is not usually confused." Yets staff still are inconsistent in using the word "delirium". If we as professionals can't get it right, we are failing families - there is no other way of describing this. #delirium

#Delirium occurs more than 1 in 3 of patients in the last days of life. Recognition matters—it allows us to address reversible causes while managing symptoms compassionately when reversal isn't possible.

In #delirium, patients rarely volunteer that they are hallucinating. Here's one useful way of asking about hallucinations: "Have you recently experienced anything strange, like visions or voices that seem very real but others don't notice?"

Myth: Sedatives (like benzos) are mostly helpful in calming delirium. Fact: Sedatives often worsen delirium. Non-drug approaches (reorientation, hydration, mobility) are first-line treatments.

Working with older people is a unique area of healthcare that requires someone to have a unique set of skills including compassion, kindness, the ability to listen and the ability to think critically.

Like others, am extremely disturbed by the revelations in the RTE Investigates programme last week. Just flagging our new book due out in August.. its now so timely.

Do you use the 'Single Question in Delirium' (SQiD) for inpatient monitoring for #delirium? Useful as a starting point - then follow up with a more definitive tool like the 4AT.
