RePHILL trial found no mortality benefit in prehospital blood vs normal saline and generated much chatter (nice lowdown on Saint Emlyn's blog). But timely reminder for “hypovolaemic shock” physiology. It is actually decompensated hypovolaemia and can be as low as 450ml loss 😩
