Things that bother me to see in intubated patients: - iatrogenic respiratory alkalosis - no BMs for days with no intervention - positive fluid balance You can’t give good ICU care if you’re not prioritizing good, detailed supportive care.

I once sketched out an algorithm for intraoperative struggling, which I'll release one day, but I'll show one of the key parts here: Basically, one of the *first* things to *always* do whenever you're struggling for any reason is to figure out if your L hand is optimized.
