Metabolic acidosis is common in preterm infants. Do you correct it? Acetate in TPN, sodium bicarbonate or no correction? Does this impact outcome? Open access Journal of Perinatology - rdcu.be/d2gXV - need for high-quality RCTs such as BASE Trial to guide evidence-based practice

An article has been published which discusses the use of base therapy for metabolic acidosis. It emphasises the need for further research on the safety and efficacy of sodium bicarbonate. BASE Trial is conducting high-quality research to assess its effects on preterm babies.

Neonatologists have been using sodium bicarbonate for 30 years without any evidence to back its use. Setting up BASE Trial has revealed the huge variation in practice in the UK but the challenge is the lack of equipoise in both directions.
