Good to be back at University Hospitals Plymouth NHS Trust again today, and thanks to Jo Beer 🌈 and Mark for welcoming Getting It Right First Time (GIRFT) UEC Team at this evening’s meeting. Really impressive engagement from clinicians across a wide range of specialties committed to improvements in the UEC pathway👍

This is exactly why GIRFT for UEC calls general and acute beds a “dominant constraint” that makes good ED patient flow very difficult to achieve, if there are too few beds for the admission load of the hospital. We must make sure that mental health is always included in the list of specialties that provide SDEC and “hot” clinic spaces. This will help to reduce the very long waits in EDs for patients with mental health problems. Getting It Right First Time (GIRFT) Royal College of Emergency Medicine

Royal College of Emergency Medicine The new pathway should mandate fewer scans than the preceding SBNS/BASS guideline (attached) as bilateral sciatica is no longer an indication for emergency MRI. The pathway was accompanied by a The Royal College of Radiologists document on extending OOH MRI provision: shorturl.at/pwyI9

Why not complete this short survey whilst on the loo? We know thanks to Chris Moulton that the number of toilets at work is associated with well-being culture. Help define a Bog Standard! Kerryn Husk forms.gle/ssYvJajMiWDqte…

The GIRFT Emergency Medicine National Report for Northern Ireland was published exactly a year ago, in response to these problems. Getting It Right First Time (GIRFT)

Thank you. I got a favourable reaction from most people who heard me talk. I was giving my usual radical message about the need for urgent change. I believe that the NHS is best improved by people who care about its patients passionately and put realism above dogma. Getting It Right First Time (GIRFT)

Our patients are suffering from a lack of timely urgent care. If a group of trained and willing people can help doctors to get their work done quickly and safely, then I am obviously in favour of it. I do not agree with PAs as substitute physicians Getting It Right First Time (GIRFT)

I only wish that it was so! In the UK, there is a shortage of almost all healthcare professionals and certainly not thousands who are looking for jobs. We have under-trained for years and have taken doctors and nurses from poor countries who need them more than we do. Getting It Right First Time (GIRFT)

If you want to campaign for something useful that would actually help underprivileged people in need, then campaign for proper wages and conditions of service for carers in the community. They do a vital job and an increase in their numbers would help the NHS so much. Getting It Right First Time (GIRFT)

The Thirlwall Inquiry is in a terrible mess as it tries to figure out whether Lucy Letby should have been stopped sooner. We now know that many of the nursing colleagues working alongside her thought she was a very good nurse and had no suspicions about her work. We also know that,
