Below “In-bed cycling plus rehabilitation significantly reduced ICU and hospital length of stay, mechanical ventilation duration and improved functional status compared to rehabilitation. Further research is needed to analyse long-term effects and standardise interventions.”

Myth: If a patient knows their name and date, they can’t be delirious. Fact: Delirium fluctuates, & even then, people with inattention can correctly answer simple orientation questions. Use onset/fluctuation, & altered alertness, & other sources to come to a diagnosis.

“My colleague was using the terminology (terminal restlessness). . . And I said, ‘‘Have we done a rectal examination? Have we done a bladder scan? . . . The colleague got back to me – even though he’d been urinating he had a bladder of 1000 mls. So they’ve put a catheter in.’

Of 30 included reports... 📌 29 from Japan 📌 Only 3 RCTs 📌 #Suvorexant in 28 📌 #Lemborexant (+/- suvorexant) in 7 🧑‍🔬 We need better trials, esp. of #daridorexant whose 8h half-life seems better suited. 🎯 I'm aiming to fill this gap. doi.org/10.1016/j.jacl…
