Tip of the day đź’ˇ: A differential cerebellar atrophy like this, should prompt a deep genetic search in the absence of a perinatal event. This child had headaches. The initial genetic screening was normal. The final discovery on an extended screen, a CACNA1A mutation. Mutations

🧠✨ Ever seen T1 bright, T2 dark thalami on MRI in an infant? That thalamic whisper could be Sandhoff disease — a devastating form of GM2 gangliosidosis. This #SpinTweetorial dives into the clinical, imaging & genetic clues. Let’s go 🧵👇 #NeuroRad #PedsNeuroRad

Tip of the day 💡: Bilateral mixed intensity subdural collections, blooming bridging veins, petechial parenchymal hemorrhages, brain parenchymal scarring….. not abusive trauma… a poor child with a complex cardiac history and recent initiation on ECMO. Context is everything.

Next in the metabolic space is the powerhouse of #pedineurorad, CESAR ALVES, from Boston Children's. A honor having him cover not just mitochondriopathies, but also their mimics. Not everything unclear is a mitochondrial disorder! Magic at #SPIN25….

Welcome to #SpinTweetorialWednesday! Today we travel into the icy silence of Salla disease, a lysosomal storage disorder where sialic acid accumulates, and white matter whispers tales of arrested myelination and destructive calm.

This week’s SPIN-POV: Progressive, confluent white matter lesions in a child, looking like a leukodystrophy? Don’t forget to rule out MOGAD—especially with a progressive-relapsing clinical course. Not all leukodystrophy-like MRIs are genetic or metabolic—some are autoimmune.

🚨 Game-changer in #PedsStroke! New paper shows mechanical thrombectomy > medical management in kids w/ acute ischemic stroke. Not on this one (ugh!) but love the brilliant team who made it happen ❤️ And Peter Sporns —your announcement was 🔥👏 #NeuroIR #Pediatrics #StrokeCare

🧠 Honored to share our insights as coauthors on optimizing #7T MRI for stereotactic planning in DBS. Precision starts with acquisition & integration—plus, we outline practical protocols for accurate targeting. A step forward for functional neurosurgery!

Uncover the key to this classic case. isnr.in/june-2025/ No bonus points for this one—but reinforcement, after all, is the key to precision.

This week’s SPIN-POV: An “L”-shaped thalamic hyperintensity on DWI/T2 points to partial prolonged HIE in term neonates—exaggerated by hypoglycemia but uncommon in isolation. In preterms, it is associated with parieto-occipital PVL, helping differentiate it from metabolic causes.
