78 y.o. female NIDCM, LVEF 28%, severe mitral regurgitation, LBBB. His-bundle resynchronization. Hisian threshold with correction of LBBB-> [email protected] ms (48 hour later). Improvement in interventricular synchrony demonstrated on UHF-ECG. Baseline and post-CRT recordings are shown

84 y.o male, complete AV block after TAVR. Permanent non selective His-bundle pacing implanted using pacemapping (His signal not visible most likely due to mechanical damage to the conduction system caused by the TAVR). Perfect synchrony on UHF-ECG. Acute threshold 0.5V @ 0.4 ms.

Previously, in a LBBA position, at the beginning of penetration, we achieved a narrow QRS, but the position was not stable. Synchrony in the UHF-ECG also improved compared to baseline, but there was delayed activation of the right ventricle.
