If I wanted to fix hospital and emergency care today, I wouldn’t spend a penny on hospitals or ERs. I would spent my entire budget overhauling nursing/rehab homes and mental health.

GI attending blew my mind today GI: acidic food doesn’t make GERD worse. Acidic food is more basic than gastric acid, so all acidic foods act as a buffer. The problem is potassium content. High K+ foods activate H/K pumps -> increase endogenous gastric acid secretion 🤯🤯

First we had an emergency room. As we provided more and more services, we grew into departments with dozens of beds. Now all our beds are filled with inpatients and we once again have a single emergency room - one that we used to call the waiting room.

News flash: your school aged child w/a cold does not need to be tested for RSV. It will not change management. There is no role for steroids or inhalers in kids w/colds, regardless of the viral cause, unless they are actively wheezing or have asthma.

Most pts in the ED with isolated HTN need no intervention at all, and the risk of iatrogenic harm far outweighs any potential benefit of acute BP lowering. That being said, initiation (or up-titration) of oral antihypertensives is easy+safe, and may improve downstream outcomes.

Because the trajectory of alcohol withdrawal is so unpredictable, effective treatment hinges on frequent reassessments. This is traditionally done based on the CIWA-Ar score, which is long/cumbersome. Using a short and easy scale encourages best practice. emupdates.com/baws
