PS: I will be writing to every CEO asking them to adopt this. I care so much about our wonderful NHS. Without the staff, it would collapse. Also happy to tweak to meet different requirements. Will let you know how it goes 😁

Can’t agree more with Simon Conroy Simon Conroy - we can’t manage a patient unless we know where they are in their life trajectory, including what matters to you. NHS Elect Improvement Networks @NHSElect #frailty2022

As part of improving our clinical frailty scoring in ED, we were thinking of providing patients in the waiting room with a questionnaire they or their carer could complete in order to generate their score. Has anyone done this already and would be willing to share? Thanks!

One of my favourite things to do is to find someone in the emergency department, put in place a plan to make them medically fit for discharge and then wheel them round to our University Hospitals of Derby and Burton NHS FT Frailty Assessment Area to begin Comprehensive Geriatric Assessment ahead of discharge.

Fantastic and innovative MDT pilot exploring the complexities of frailty and the care of older adults! Learning through simulation provides a safe, growth mindset environment so skills can be put into practice. Great work Sim faculty and candidates!! Deborah Sanders RN💙 Bev Thomas

We discussed this exact statement in our Frailty MDT simulation training yesterday, so important to use our listening skills to really understand our patients and their families @KVangucci Rosie Skrypak @SuzanneScaffar1 Martin Glasser
