On randomized trials (RCTs). My thoughts as an ardent proponent & having led many. 1) Easy to call for an RCT. Hard to do one. 2) For every 100 questions that an RCT is needed, we have funding for 1. 3) Designing, securing approval, & successfully completing an RCT is hard

At long last, here is part 5️⃣ in my animal #physiology in the ICU series #FOAMed Case 1: You are called about a very high blood glucose (BG) value: 738 mg/dL (41 mM)! The RN asks if you want to start an insulin drip. You say no, it’s totally normal because the patient is a: 1/

Part 6️⃣ in my extremes of animal physiology #tweetorial series: Case 1: How low can PaO2 go? You are called by the lab about an abnormal ABG result. pH 7.5 / PaCO2 20 / PaO2 22 / HCO3 15 “That’s gotta be venous” they say. “No” you reply, “Its from a ___" 1/

1/13 🤔Why does multiple myeloma cause anemia and not pancytopenia? If the issue is infiltration of the bone marrow with plasma cells, wouldn't all cell lines be low? Let's have a look.

1/12 🤔Why does iron deficiency cause fatigue, even in the absence of anemia? I have often equated iron with hemoglobin and oxygen-carrying capacity. But there is so much more to it!

1/12 - Mystery #1 You are seeing a patient recently diagnosed with heart failure and started on GDMT. You notice that their hemoglobin (HGB) has increased (12 → 13 g/dL) in the intervening weeks. 🤔Which medication is the likely cause of this increase in HGB?
