Cyril Zakka, MD Beyond the long-term educational issues, this also really validates the concerns from our NEJM editorial Adam Rodman Arjun (Raj) Manrai. Divorcing reasoning from documentation changes the underlying cognitive process, and we can't assume without evidence that they are equivalent.

I hear interns and residents banally say they had to prioritise due to time limitations so they couldn't do a full note. I see the use of copy-paste. I feel clinical reasoning skills are changing (to put it politely). I fear what might be next. #MedEd needs to adapt at all levels
