How many inquests lead to PFDs which touch on issues where lessons should have been learned from previous inquests and previous PFDs? Answer: too many - there is a very good argument for a National Oversight Mechanism.

We really have heard all of this before. Will this be the time it actually happens? The majority of the budget increase will likely go on pay, not transformation.

Can we be careful, please, not to blame kids and their families for the SEND crisis?

Yes more resources & funding would be good, but this is about culture. NHS England & DHSC don’t seem to understand this. MH Hospitals could have all the funding in the world & still treat people with the same indifference, disdain & total lack of compassion.

The CQC cost circa £400 million a year, £40 million from DHSC, the rest from the institutions they inspect. No wonder they don’t want to bite the hand that feeds them……..it’s time for change, massive measurable change.

The head of the CQC has been meeting user groups for only the last 6 weeks........... (started there 2 years ago) what have you learnt and what happened for the decade before? The CQC has had its day, root and branch change required to give measurable improvements.

While I mostly agree✔️ I am pretty sure I wouldn’t be handing it to Drs without a safeguard/check & A An Other professional in place. In general terms & in my experience. Drs under P r pretty poor at making time & decision specific capacity assessments - for lots of reasons>
