Sometimes the little extras make the difference in getting the line or not getting the line. This not a how to. These are some tips and tricks that people have taught me over the last 2-3 years, and that I teach to residents. All additions, corrections, and comments welcome!

Good critical care is 10% performing dramatic interventions, 90% being aggressively persistent with the seemingly mundane details. - Fluid balance - Weaning sedation - Weaning the vent - BMs - Deescalation of lines and tubes - Deescalation of abx/other meds

1/In lieu of Meet Up Monday, this week due to the holiday, we are doing...Tisha Tuesday! So without further fanfare, though she deserves it all, why don't we meet the indomitable, Tisha Wang our APD.

A non-medical friend recently asked me to describe clinic. I told her to imagine you have 20 meetings in a day, half of them new clients with urgent needs. Each requires your best self. You are late for at least 10 of them. You must prepare a report and deliverables for each one.

When the doc you only see occasionally walks into the room and asks what pair of rothys you’re wearing today, you might have a very specific shoe problem. Can’t stop, won’t stop!

You can table round. You can bedside round. You can do systems based, you can do problem based. Gravity rounds, bottom of the list up rounds, new patients first, old patients first, whatever you want. All I want is organized/succinct and don’t skip FASTHUGSBID.

I often teach the a few core principles of what makes ICU decision making different from treating what appears to be similar problems on the med-surg floors. Here are a few principles I emphasize, in no particular order...🧵

Ten cognitive check points (general rules or heuristics to help guide my practice) I use in the ICU to help make sure I don't get too far off track. A thread🧵👇 1/10: Shock with a narrow pulse pressure (<30mmHg) needs an urgent #POCUS (or echo) to identify occult RV/LV

When I’m supervising, I expect people to - be reliable - be considerate - be truthful - be organized - take ownership - be self motivated - have plans
