Excellent, excellent paper and moving towards what I have been advocating for years. While I am skeptical of some of the suggested approaches, the fundamental point is spot on, ie that we need to put the puzzle pieces 🧩 together of the wide array of knowns and unknowns and

Second great read today. Many apt observations, including this: “The din and cacophony of researchers barking up the wrong tree sometimes drowned out the news of real progress.”

Congratulations to NYU Langone for betting on the perfect combination of skill, passion, intellectual curiosity, and a genuine “desire to make a real difference in the lives of children with IBD”. Few things give me true hope. This does.

Haven’t thought about this in a while but do we have hypotheses about why only a subset (20-40%?) of Crohn’s patients have granulomas?

Dr Mark Silverberg Ildiko ☕️ Changes in phenotypes are so evident that even a single clinician can see them (as we do) in some 10 years (more clearly in 40 years). Not surprising if one knows a bit on biology (some books worth to read below): Cajal’s ideas were right after all

This is very good. I would still personalize, especially for IBD patients who likely require modifications but overall I am always appreciative and supportive of this type pf much needed work.
