@IslingtonCCG Islington GP Federation QI team have more than doubled (107%) the number of people at the end of life who have a CMC care plan in place. This is projected to save £530,774 by preventing in-hospital deaths. This is such a huge achievement, thank you for all of your hard work.

Our QI team have supported our excellent nurses, dually qualified in physical and mental health - to increase the number of patients with severe mental illness who have an annual health check and receive high quality care planning as part of this review @IslingtonCCG

Our QI team worked with a #primarycarenetwork in Islington and managed to achieve improved vaccination rates in all eligible groups, despite vaccine uptake being lower across London compared to 2018/19 Shaine Mehta
