Kidney on fire: Persistent UTI's for last 2 years (with constant antibiotic exposure) provided kindling for the wildfire... now there is severe pyelonephritis (ouch!!). Interestingly also eosinophil rich areas, possibly related to prolonged antibiotic exposure...

Checkpoint Checkmate: It's been a little while since I've seen this, tubulointerstitial nephritis due to immune checkpoint inhibitors. Typically the "sweet spot" (at least in my observations) seems to be three to four months out, but this patient developed AIN after 8 months.
