Madendere (Prostate 2024) found 19% of initial neg biopsies for PI-RADS (P) 4-5 had persistent PI-RADS 4-5. With repeat biopsy, 25% had csPCa. My comment: Here differentiating high-risk from low-risk PI-RADS 4-5 is crucial. Clinical data, (zonal) PSAD, and contrast-MRI may help

This is the reason, why ultra-high b-value images (e.g. b3000) are more specific for cancer in the TZ. An observation I have seen in my patients since I have been using b 3000 (calculated). '
