The NIH has decided that scientists can only submit 6 grants a year. OK. But you need a score that’s < 10% at least to get funded. Meaning, at best, 1 out of 10 grants you submit will have a chance (YMMV of course). 🫠

It not a statement of modern healthcare systems in general, but that of the US healthcare system. Performance metrics for Canada are either the same or better than the US and they spend abut 29% of what the US spends on administration. ( NEJM 2003 349(8): 768-775)

If NIH payline is 10% that doesn’t mean you have a 1 in 10 chance. It means if your grant isn’t better than 90% of others, it gets nothing. Write an average grant? You get nothing. Write 1000 average grants? You still get nothing. It’s not roulette. It’s a ranking.
