Cardiovascular disease (CVD) is responsible for 25% of UK premature deaths. Up to 80% of these deaths are preventable. It’s estimated that the burden of CVD costs £29 billion each year. But CVD prevention is not currently being prioritised by government at the scale needed.

We've been trialling a similar programme to ensure people affected by stroke know our services from the point of diagnosis. So, we're confident that Diagnosis Connect can help transform early stages of recovery and rehabilitation for the 1.4 million stroke survivors in the UK.3/3
