There are 2 reasons why things don’t happen. ā€˜I don’t know’ or ā€˜I don’t want to’. And there may be very good reasons for the latter. Why does the HEE not increase the training places in geriatric medicine? Or is the uptake poor? We need a honest discussion.

Well done to our Acute Frailty Team who won a Patient Safety Congress & Awards award. The AFN have worked with 144 hospitals to improve the care of older people, leading to improvements like reduced hospital stays so people can get home sooner safely. Lucy Pollock We should be allowed to use some indidualised decision making. Have made this point for this scenario in an upcoming GP TARGET video. Expecting some controversy…

Absolutely! When do we as an NHS, stop focussing on the ā€˜symptoms’ of flow issues and address the out of hospital resources, processes and 24/7 availability? Short term fixes are NOT sustainable

How many of them could have been managed at home with the right conversations and appropriate resources? Could we do any better?

stemlynsblog.org/speaking-urgen… the circled bit is what stops FLOW IN EDs. This is work that gets duplicated/ triplicated. Specialists need to see them in their assessment units at point ā€˜0’ and not 6- 24 hours later. Focus on getting speciality footprint, opening hours right.

Every little helps in a complex system. No single magic bullet. What we want is change in processes to deliver this hour’s work this hour or at least today’s work today for UEC.

Congratulations @sheddoc Let’s continue the journey and get fitness and flow in frailty. Looking forward to working with you.

Agree Brian Dolan OBE, RMN, RGN regarding age based cutoffs. Just a surrogate marker until we get better at scoring frailty which I must say we are getting better at. Otherwise we have very little data to understand the demand and create the best services for our frail population
