several caveats. Gamma/delta T cells (often CD4-CD8-), regardless clonal or not clonal, will not express TRBC1/2; When CD3 is negative/dim, TCR expression will be truncated, the TRBC1 can be weak or even negative.

Blood smear with classic reactive lymphocytosis (EBV-driven infectious mononucleosis). The Downey classification system is useful for recognizing the wide variety of lymphocyte morphologies typical of a reactive lymphocytosis. Watch my #CAP20virtual course for more! #hemepath

#Hemepath pitfall alert! Thrombopoietin-mimetic agents such as eltrombopag (Promacta) & romiplostim (Nplate) cause changes in megakaryocytes that exactly mimic MDS/MPNs. The paper below describes some abnormal forms, but I've seen the whole gamut. Be wary! ncbi.nlm.nih.gov/pmc/articles/P…

3:1 RULE VIOLATION I posted a poll asking how you would treat a patient with a Hb 5.5 and Hct 20.2. The correct - and most popular - answer was iron. The clue is a violation in the 3:1 rule, which is another way of saying the MCHC is abnormal, in this case greatly decreased.
