#ISC25 ETERNAL-LVO 🇦🇺 (IVT <24hr in pts with LVO and perfusion mismatch) randomized trial showed no benefit. post-hoc subgroup analyses does seem to show a signal worth exploring of benefit in pts who presented to spoke sites requiring transfer.

Another (+) trial from China with HOPE: IVT between 4.5h-24hr #ISC25. HOPE enrolled LVOs but also (presumably) medium/small vessel strokes if perfusion deficit was present with small core. ▪️ Not published yet, interested in parsing out population differences vs TRACE-3.

Following the (+) adjunct IAT study for ANGEL-TNK, another Chinese study (PEARL) is positive for benefit in 90d mRS with IA TNK after EVT. #ISC25 🔑major diff from ANGEL-TNK is enrollment of pts 0-4.5hr including if they received guideline IVT. 👍again, no major ICH signal

#ISC25 I personally appreciate the randomized ATIS-NVAF as whether to use ASA+DOAC vs DOAC in an afib pt w/ atherosclerosis hx like CAD or carotid stenosis is an eternal rounding question. 💊monotx doac less bleed risk, and no incr in CV events 🥺limitation: variability in tx

#ISC25 LAMP study evaluating liraglutide in high risk Tia or minor ischemic stroke with T2DM suggests benefit but is unfortunately underpowered due to early termination. Need more followup studies.

Last but not least at #ISC25, results of phase 2 BEST study (BXOS110 neuroprotectant novel inhibitor of PSD-95 protein alongside reperfusion therapy w/n 3hr) showed sig effect on proportion of pts with good func outcome (mRS 0-2) with high dose. But increased adverse effects.

Another great #ISC25 coming to a close. While reflecting, I decided to make a series of reaction memes to capture my personal reactions to this year's major presentations.
