Pathogenic Germline Variants (PGVs) in cancer genes (e.g., BRCA1/2, PALB2) are well known to increase the risk of certain tumors, such as breast cancer. Testing is recommended in high-risk cases. ❓But are they routinely assessed in patients with LC?

Over 20 years, 702 families were identified as PGV carriers after genetic counseling at the Hospital Clínic Interestingly, 15% of these PGV+ families had LC cases—significantly higher than in non-PGV families.

Most families with LC cases carried PGVs in homologous recombination genes, followed by mismatch repair genes and CDKN2A variants

Despite indication for germline testing, only 13% (17/130) of patients with LC from PGV+ families underwent testing with 86.7% PGV positivity🚩

Patients with LC and confirmed PGV (n=15): 👉Mostly male (80%) 👉Median age 60 yrs 👉89% smokers 👉Adenocarcinoma (3/5), squamous (2/5) 👉All analyzed tumors had actionable mutations (KRAS, EGFR)‼️

Zooming out: This real-world study shows significant LC prevalence in PGV carriers' families, especially those with DNA repair gene variants. Yet germline testing remains rare, underscoring the urgent need for clear genetic testing guidelines in LC 🫁🧬 #LCSM #Genetics
