Love this…didn’t we use to call those ‘echo beats’…? P waves not regular because the retrograde ‘P’ resets the sinus node. Also the P wave buried in the terminal aspect of the third QRS doesn’t have the same morphology as the others: it’s negative in lead II (a pseudo ‘S’)!

Completely agree. Underrated. Underused. Physiologically makes sense. Certainly the best data (in my opinion) for interpreting AS in what is a phenomenally challenging group of patients.

Ok on this note…what are the best examination features of severe AS? I’ll go first… I was once taught my someone ‘old school’ to feel for apical-carotid delay. I’d recommend it. If you can feel maximal apical pulsation BEFORE the carotid it’s always severe AS. Simples
