4 - In 2014, with the support of the World Association of Sarcoidosis and Other Granulomatous Diseases (WASOG), an updated version of the ACCESS sarcoidosis organ assessment instrument was published. WASOG pubmed.ncbi.nlm.nih.gov/24751450/

5 - Also in 2014, the HRS published the HRS expert consensus statement on the diagnosis and management of arrhythmias associated with cardiac sarcoidosis. heartrhythmjournal.com/article/S1547-…

6 - In 2015, the Genomic Research in Alpha-1 Antitrypsin Deficiency and Sarcoidosis (GRADS) Study protocol proposed diagnostic criteria for cardiac sarcoidosis. pubmed.ncbi.nlm.nih.gov/26193069/

7 - Finally in 2016, the Japanese Circulation Society proposed a guideline that includes criteria to clinically diagnose cardiac sarcoidosis: jstage.jst.go.jp/article/circj/…

Of the 7 criteria, only the JSSOG 2006 and the JCS 2016 criteria allow for a clinical diagnosis of cardiac sarcoidosis without any extracardiac tissue proof of sarcoidosis.

Of these 7 sets of criteria, only the JMHW 1993 criteria were based on cases of definitive cardiac sarcoidosis (proven by endomyocardial biopsy or autopsy). The rest were based on expert consensus. None have been validated against cardiac pathology data after they were proposed.

We have many criteria... how do they compare against each other? Ribeiro Neto et al. from the Cleveland Clinic examined 69 patients judged to have cardiac sarcoidosis by their multidisciplinary sarcoidosis team and compared the WASOG, HRS, and JCS criteria...

Ueberham et al. from the University of Leipzig examined 62 patients with suspected cardiac sarcoidosis and compared the WASOG, HRS, and JCS criteria. They also found a substantial disagreement among the current diagnostic recommendations. sciencedirect.com/science/articl…

In an abstract from the Cardiac Sarcoidosis Consortium presented at ACC 2022, only 60 of 587 patients met all 4 of JMHW 1993, JSSOG 2006, HRS 2014, and JCS 2016 criteria. The two Japanese criteria were more likely to identify patients who experienced an adverse cardiac event.
