Always be prepared for the unexpected during endothermal septotomy. In this case we correctly predicted that the septum might cover the LRA after fenestration so we pre-stented; then hot wire through it via the fen. #aorrtaEd

Our setup for TAAA with VA ECMO. 14mm intercostal graft for arterial cannulation, R femoral vein cannulation, Fogarty catheters in the intercostals to be preserved. Next step - off ECMO, divide 14 mm graft and sew it to intercostals. Total transfusion 1 unit PRBC. #aortaEd
