🚨APPEAL trial results #EAU25 by Kari Tikkinen. Largest most robust multicentre RCT to date evaluating if antibiotic prophylaxis reduces UTI in ESWL (n=1722). Cipro (vs placebo) ⬇️ urosepsis 0 vs 1.2%, p=0.003 (tertiary outcome). No stat sig diff for primary or symptomatic UTI.

Congratulations Kari Tikkinen - amazing to conduct such a high quality robust trial. Lots to think about. Small benefit in serious infections; but not for simple UTI. Is that benefit enough to treat all? How does this fit with antibiotic stewardship ? How might we risk stratify?
