The major issue in 🌍 & 🇺🇸 is lack of subspecilization of this surgery, where patients trade of their medical complications to surgical one.

If someone wants to live with their ileostomy, it’s ok. At the same time, this is uncommon, and most of the people would like to have pouch surgery

It is the duty of the IBD GI , referring their patients only to experts who does these surgeries at least several times a week .

Kotze Fernando Gomollón Phil Smith If someone does not want a J Pouch out of circuit Rectum needs to be surveillances, depending on the duration of the symptoms. Anyone over 10 years I do recommend yearly.

Patients need to ask surgeons their experience and no junior faculty should do a J Pouch surgery by themselves right after the training unless it’s double teamed by another senior Surgeon who has experience.
Leaving mesorectum behind and putting pouch through intramesentery , creates collar around the intramesenteric anastomosis that results in stricture , vaginal / fistulazation and patients gets labeled as Crohn’s for mechanical/ surgical issue.. Northwell Health
