Michele Introna A lot of dosing is a legacy of the past: keeping math easy & counting drips. EHRs & computerized pumps made IV infusions easier & safer, but the underlying calculations are probably too simple. We are starting to use better algorithms for Vanco dosing. Hopefully other meds soon
