Dr Shawn Walker #OptiBreech Emma Spillane ๐ŸŒˆ Good plug for the fabulous Breech work the safety enhancements brought about through using the algorithms and prompt sheets in practice ๐Ÿ‘๐Ÿ˜ Fabulous feedback from all when using in real practice . Thanks ๐Ÿ™

Greatest danger is when the buttocks are on the perineum / rumping. Delay at this point causes hypoxia, leads to poor tone, can result in arm/head extension. โณcrucial. But this is the point when HCPs are MOST likely to chant "Hands off the breech!"๐Ÿ˜ฉ

There is absolutely ZERO evidence that touching the baby at this point will cause a startle response leading to extended arms or head. ZERO. Folks, do the VEs you need to do to assess safe progress. Sweep the perineum or cut an epis (w/consent) if needed.

Flexi sacrum positions, kneeling, standing, squatting & sidelying may reduce instrumental births, cs, epis, severe perineal trauma, severe pain & shorten pushing phase. Yet 83% give birth semi recumbent or supine. Best birth position chosen instinctively researchgate.net/publication/34โ€ฆ

As Ockenden Report makes clear, there is danger when training does not match guidelines. If you donโ€™t teach people how to safely do something their guideline says they can do, e.g. upright breech, they will teach themselves with buzz words, โ€œHands off!โ€

Congratulations @HomebirthTeam, Louisa Davidson and family! #OptiBreech 1 is an observational study, following women who choose to plan a vaginal breech birth -- wherever they choose to plan it. Glad to hear of this wonderful outcome Bham Women's Hosp ๐Ÿ’™ Emma Spillane ๐ŸŒˆ

This โฌ‡๏ธ Over medicalisation of childbirth is causing avoidable and unnecessary harm both physically and psychologically to mothers and babies.

New stats just released for place of birth in 2020 confirm what many of us witnessed on the ground - a rise in the homebirth rate during the pandemic. Highest rate since 2011. ๐Ÿงต
