Subacute bilateral (L>R) SDH w midline shift. A case: spinal bone spur-> dural tear-> CSF leak -> brain sagging -> traction on the bridging veins -> SDH. Pearl: Subacute SDH (I.e. day 3-21) can be isodense to the adjacent cortex (35-40 HU) and can be missed esp. if bilateral.
