From my Covid sick bed, a short thread on the utter nonsense of framing public health as a matter of 'personal responsibility' 1/4

It’s been a week since our Pathway conference and it’s taken me since then to process it. Being with a bunch of people who talk seriously about concepts like love, care & hope, and combine it with a serious evidence-based commitment to make things better was so powerful. ✊🏻

Ideas for finals OSCEs next year 1. Give the student an illegal rota and challenge them to sort it 2. Fix a malfunctioning printer in the 8 minutes before WR 3. Acquire a CTAP with no other indication than 'the reg wants it' #MedTwitter am I missing any?

I spoke to the RCP health inequalities series about the basics junior docs need to know about caring for people who use drugs and alcohol in hospital . Feedback from twitterverse greatly appreciated, all views my own open.spotify.com/episode/3TFO8V…

We reviewed "substance dependence management" policies at acute hospitals in England - new study in BMC Medicine. These policies are inconsistent, stigmatising, and may prevent timely medicine for opioid withdrawal. Open access: bmcmedicine.biomedcentral.com/articles/10.11…. Some examples... (1/10)

Hot off the press, we reviewed the policies of 86 acute trusts, and found them to a stigmatising and inconsistent. We must improve the way we support opioid withdrawal in hospitals bmcmedicine.biomedcentral.com/articles/10.11…

Haven’t tweeted on hepatitis as I’m not directly involved but I see a lot of v unhelpful speculation going on and, sadly, people accusing public health bodies of bias. In a vacuum of answers, people will find their own. The fact is: we just don’t have a definitive answer yet.

We found that multicomponent interventions and interventions that focused on education & empowerment were of benefit. Interventions that used peer design & peer delivery were effective. Sex workers (SWs) new to working in an area faced greater challenges accessing services. 2/4

Data were scarce for male, transgender, & indoor-based sex workers (many studies looked exclusively at female, street-based SWs). There were also major gaps in research related to chronic disease and reproductive health. 3/4

Luke Johnson, MD, MSc, FFPH (@luke_a_johnson) 's Twitter Profile Photo

Implications include a need to develop & deliver interventions in collaboration with SWs, focusing on multicomponent interventions which empower SWs. Inclusion of chronic disease prevention/treatment should be considered. A gender-sensitive and inclusive approach is key. 4/4

Paper published in The Lancet Public Health yesterday about sex worker health, reviewing affective interventions. Under researched area with great potential for affective peer designed interventions and outreach. Need for greater inclusion . Full article linked doi.org/10.1016/S2468-…

Our recent study of COPD among people who use heroin. Diagnosis is often late, and the risk of exacerbations is double that of other patients.

Health inequalities related to economic disadvantage is not a small leftfield subject, and the populations are large and growing. We can thank years and years of austerity for that. (5)
