Missing delirium is bad. Delirium detection should not be left just to champions/enthusiasts. We need to upgrade healthcare system norms to make it that we always look for delirium in high risk populations. And when we don't even try it should be considered BAD PRACTICE.

Human kindness can significantly reduce the distress associated with #delirium. Eye contact, gentle words, reassurance, explanation of context - simple and quick to do, and very effective in improving the experience of the delirious patient. Build this into every interaction.

Every hospital has pathways for recognition & management of acute coronary syndromes and stroke - but why not #delirium, a condition with a higher 30-day mortality?
