NOM for early acute appendicitis 90% effective (if you don't have a faecolith) = discharge with appx in your abdomen still. 80% of that 90% still have their appx at 12 months = 72% overall. So around 1 in 4 fail within a year. It's plan B for me, not plan A

Majority of complications are re-presentation to hospital with parental or child concerns. No intervention required. To me that's more a failure of pre-discharge explanation. Happy to see them again, to catch the few with a late intra-abdo collection/abscess

Christian 🇨🇦🌲 Whether a patient is stable with any BP is a judgment call but someone could definitely be 95/60 and stable. Here, with one word -“stable”- you are communicating to the MDs that your judgment is that pt. is fine. Very efficient communication.

Except that, for some people 'stable' means unchanged. They may have just looked at the trends & seen no alteration. It doesn't necessarily mean they've applied any more thought to interpretation. Dead patients have 'stable' obs (other than downward trending temp)

On call last night at the children's hospital. 5 sparkler burns in 2-5yo children in 5 separate incidents between 6 and 9pm. 5 burnt hands we're now hoping don't scar. So unnecessary.

At a time when it was unusual for fathers to be in the delivery room, he was there I drew my first breath. It was a privilege to be there with Dad when he drew his last. He was my rock, my safe harbour. My siblings & I will be ok, thanks to Mum and Dad. But we will miss him.
