DESMOPLASTIC SMALL ROUND CELL TUMOUR. IHC: desmin. NGS: EWSR1::WT1. NB: sheets of polygonal-spindle cells; eosinophilic cytoplasm; monomorphic round-ovoid nuclei; occasional thin collagen bands; ~ one third of cases lack 'conventional' histological features (PMID: 9808123).

🔬 New case report: Rectal malakoplakia identified incidentally in a Crohns patient highlights diagnostic challenges in IBD pathology. This rare finding reminds us to maintain a high index of suspicion and consider malakoplakia in atypical presentations. #PathTwitter #GIpath

I know I've posted about this relatively recently, but this was too pretty to pass over. X10 for orientation, x40 for detail What IHC would be positive in the nuclei?

⚡️ SPEP ➡️ SERUM Protein Electrophoresis. 🤯 Serum > 1,500 circulating proteins ⚠️ But NO Fibrinogen. 🤔 If you do Electrophoresis w PLASMA --> Fibrinogen can create a spike and can👀 like M Spike🦴 🧪 You can do IFE to verify, but that’s why you do SPEP not “PPEP”

NOT a lymph node FNA, this spectacular cell block is from a pleural effusion of a patient with known history of pulmonary tuberculosis. I've rarely if ever seen that many "necrotizing granulomas" in an effusion sample, some with the classic Langhans giant cells.

Pathology routine, or as they say — fear often has big eyes. A tumor like this, seemingly obstructing the lumen of the sigmoid colon and measuring about 5 cm in diameter, turned out to be just a lipoma that had simply pushed the epithelium above it.

Currently in Army 🪖 basic training —learning a lot outside of pathology but some principles connect. One phrase stood out: "Better to learn it and not need it than to need it and not know it." It reminded me how in pathology, why we learn the rare 🦓 (Views - my own)
