Exile by Taylor Swift & Justin Vernon #DocsWhoRock #RegretWeek Late (re-)post of remixed version I pulled it down Fri nite as I wasn't satisfied w/ mix (still not), but our charge RN asked me to repost And everyone #ICU doctor knows you *must* obey the charge RN :)

These are lungs (most recent CXR) of 12 sickest #COVID19 patients I have taken care of in last 24 hours in our #ICU Despite hours of trying to outwit C19 & tweak ventilators just right, I am more often losing against virus. This is hard; we are so often helpless.

The brains, heart, & backbone of an #ICU: The bedside nurses Our ICU nurses are truly amazing & inspirational. If you really consider them heroes, please read their reflections on their work. This article echoes the sentiments of anyone working in a #COVID19 ICU right now.

There has been a surge of infections in the US and across much of Europe At one point, both UK & France had more cases (per capita) than we did But the three big European nations put in policies to bring the virus under control And its working And we haven't And it shows

Finally, we see some RCT data on #tocilizumab And shockingly, the news isn't good More proof that #COVID19 is best managed through prevention... And for the symptomatic, mostly supportive care nejm.org/doi/full/10.10…

While anosmia is improving, I am quite certain I have a peripheral neuropathy post-#COVID19 / #LongCovid I feel for everyone with chronic fatigue, idiopathic small fiber neuropathies, fibromylagia etc, many of which have likely always been postviral syndromes

This is my first tweet in 536 days. Thanks to this awesome crew of internists-in-training for such an honor. It's been a great first 14 months on the job here :)

OK I finally will buy into the idea that #VExUS score is at least prognositically useful In my experience, it is also *probably* diagnostically useful for phenotyping #cardiorenal & therapeutically useful for diuretic dosing #POCUS onlinejase.com/article/S0894-…

Before deleting this photo forever to make room in my Google account, here's my fav case *ever* of using esoph manometry for PEEP titration Airway closure was present < 20 cm water Look at this end-expiratory PL: Right at 0! Who else has delivered 30 cm PEEP with FiO2 0.25?

I'm biased toward #thiamine supplementation IMHO, recent RCTs have been small, underpowered, & maybe misinterpreted by frequentists (e.g. TRPSS study pubmed.ncbi.nlm.nih.gov/37364280/) So I embrace this latent class analysis w/ ↑survival in sickest #sepsis pts pubmed.ncbi.nlm.nih.gov/39614357/

I have received a number of memorable end-of-rotation evaluations, thank you cards, etc over the years However, I really got a kick out of this modified quotation lifted from the Red Rising saga left in the resident workroom Very fair summary :)

Just a little Friday PD-to-APD banter loosely related to our future residency program... But clearly a glimpse into our personalities And a preview of what our future #internalmedicine residents can expect at #morningreport :)
