Concerned about encouraging the unsteady patient to walk in case they fall? They'll only become more unsteady the longer you wait. The clinical evidence is in your favour to take the risk. #Deconditioning = harm, despite the perceived lack of 'trauma'. #endPJparalysis

Kate Masters EndPJparalysis - Brian Dolan OBE, RMN, RGN Elective & Emergency Care Improvement Support Team Brian Dolan OBE, RMN, RGN One of saddest sights for me is frail patients having to spend their last 100 days in hospital because nowhere for them to go & be well cared for in a homelike setting. Delayed Discharges can deny people of their right to a calm loved non clinical end of life. Kathryn Mannix

Pathfinder has demonstrated that there is a safe alternative to ED conveyance for older people following low-acuity EMS calls. A model which could be spread, with local adaptation, nationally & internationally. Read this article by members of IFEM GEMSIG: ifem.cc/pathfinder

Comprehensive geriatric assessments allow for identification of the multidimensional aspects of older patients care. Use of a CGA allows more accurate diagnosis as well as making a treatment plan that takes into account what matters most to those we treat! posters.geriemeurope.eu/posters/p01/

Waiting for the discharge to be safe❓ Waiting for the physio ax before 'allowing' the patient to walk❓ Waiting for the results of one more test before they can go home❓ If you wait for zero risk. The patient never stops waiting❗#EndPjparalysis

Hospital acquired deconditioning (HAD) is often an avoidable harm that we can manage better! Why aren't we measured on this. And if we were do you think HAD would reduce? Resulting in improved function, reduced delirium, maintained muscle mass, and more ppl returning home
