Since we are here: do you have high quality evidence (like GRADE methodology you mentioned) to show that the benefits for these treatment strategies outweigh harms? Do you have high quality evidence on the accuracy of diagnosis (again, following GRADE methodology)?

There is no contradiction between these two statements: 1. Clinical research should be scrutinized and, if needed, criticized for its flaws. 2. Better-designed studies should be opposed when plausible clinical & ethical rationales for intervention have not been articulated.

quality of evidence is essential. The systematic review needs to tell not only the effect estimates, but also how confident we should be about the effects. The Utah review FAILs to assess the quality/certainty of evidence, using stronger language than it should be.

Review authors can say anything about effect estimates. We should only trust reviews using scientific methods to assess how certain the effect estimates are. The Utah review does not include this process, and its conclusion should NOT be taken as it is without critical appraisal.
